چكيده : جهت بررسي ارزش سونوگرافي واژينال در تشخيص نوع كيستهاي تخمدان، از راه مقايسه آن با جواب سيتولوژي مايع درون كيست از طريق تخليه از راه واژن اين مطالعه انجام شده است . با توجه به اينكه كيستهاي تخمدان در صورت باقي ماندن ممكن است دچار عوارضي مثل چرخش يا پارگي شوند و از طرفي دخالت زودرس جراحي ميتواند موجب حذف تخمدان و يا چسبندگيهاي لگني و بدنبال آن ناباروري گردد، در سالهاي اخير توجه به انجام پونكسيون اين كيستها از طريق سونوگرافي واژينال معطوف شده است ، بطوريكه در يك مطالعه روي 278 مورد، آسپيراسيون كيست با تحت نظر قرار دادن بيماران بدون اعمال جراحي نظير لاپاراتومي مقايسه شده است و نتيجه آن حذف حداقل 50 درصد موارد جراحي بوده است . در اين تحقيق طي مدت 9 ماه در 82 زن كه كيست تخمدان آنها با مصرف سه ماهه قرص جلوگيري باقيمانده، از راه واژن با هدايت سونوگرافي تحت عمل آسپيراسيون مايع كيست قرار گرفته و طي آن منظره سونوگرافي و سپس رنگ ، ميزان مايع آسپره شده و نهايتا جواب سيتولوژي مايع كيست مورد بررسي قرار گرفته است . نتايج حاصله نشان داد كه صحت تشخيصي (Accuracy) سونوگرافي در مورد كيست فانكشنال 94/9 درصد، در مورد آندومتريوما 97/5 درصد و در مورد كيست اپيتليالي خوشخيم 97/5 درصد است . همچنين اندازه كيست تاثير مهمي در نوع و ماهيت آن نداشته است (زير 10 سانتيمتر) ولي رنگ مايع آسپيره شده در مورد آندومتريوما مؤيد آن ميباشد. اين مطالعه پيشنهاد ميكند كه بجاي دخالت سريع جراحي، با تشخيص كيستهاي تخمدان به كمك سونوگرافي، تحمل و يا آسپيراسيون كيست انجام شود.
